Job summary

Pembroke Hospital is seeking an Intake Coordinator for a Part Time, weekend schedule (2pm-10:30pm, 24 hours per week). The role involves handling inpatient referrals, gathering demographic, clinical and insurance information, and ensuring accurate data transmission to clinical and business office staff.

Responsibilities include coordinating admissions, communicating bed availability, and supporting timely referrals.

Responsibilities

• Completes the intake call sheet on all referrals including for inpatient as appropriate for the position.
• Gathers demographic, clinical and financial information on call sheet.
• Ensures information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff.
• Requests evaluation from ESP/ED/Referrer as appropriate.
• Assists Intake Coordinators when requested in interpretation of lab values and diagnostic testing.
