Job summary

Horizon Health Behavioral Health Services in Neenah, WI, is seeking an Intake Assessment Coordinator (Part Time) to support the inpatient psychiatric unit. The role coordinates referral development and pre-admission patient assessments under clinical supervision.

Shifts include weekday evenings and weekend coverage; typical schedule 24 hours weekly with Saturdays or Sundays 9 AM–5 PM. The position emphasizes collaboration with RN, LPC, LMSW and the interdisciplinary team.

Qualifications

• Bachelor’s Degree required.
• 3-5 years of related work experience.
• Proficient in software applications.
• Ability to read and comprehend moderate to complex instructions and correspondence.
• Ability to effectively present information in one-on-one and small group situations to patients and their families, facility leadership, employees’ clients, providers, vendors, etc.
• Ability to write detailed correspondence.
• Excellent verbal and written communications skills.
• Ability to Prioritize Multiple Tasks.

Responsibilities

• Responds to intake calls and completes admission paperwork.
• Facilitates groups, family and individual sessions and completing all necessary documentation.
• Schedules Unit assessments, meetings and appointments as appropriate.
• Utilizes assessment tools under authorization/supervision of RN, LPC or LMSW.
• Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
• Reviews newly admitted patient accounts for eligibility and authorization daily.
• Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.
• Participates as an active member of the interdisciplinary team.
• Addresses issues in a timely manner including crisis and higher level of care referrals.
• Performs other duties as assigned/required by this position.
