Job summary

Empath Health is seeking a RN leader to provide daily oversight of the Access Team within Admission-Care Navigation in southwest Florida. You will liaise with hospitals, uphold regulatory and accreditation standards, and optimize patient access and care navigation for quality and continuity.

The role emphasizes staff coaching, training, and performance feedback, with travel between multiple locations and a focus on compassionate Full Life Care for All.

Qualifications

• AS degree required; Bachelor's degree preferred; Master's degree preferred.
• Two years hospice experience with at least two years of management experience preferred.
• At least five years total RN experience preferred.
• RN licensure and ability to travel between locations.

Responsibilities

• Provide daily leadership, oversight, and support to the Access Team within Admission-Care Navigation.
• Serve as liaison to hospitals, communicating Empath Health's mission and values.
• Ensure compliance with state/federal regulations and accrediting standards.
• Coordinate access and care navigation processes to promote quality and continuity.
• Monitor performance and develop staff through coaching, training, and feedback.
