Suncoast Hospice of Hillsborough, a member of Empath Health, seeks a Hospital Access Navigator in Hillsborough County to lead the Access Team and ensure smooth entry to care pathways. You will liaison with hospitals, uphold regulatory standards, and oversee coordination of patient access and navigation processes across the care continuum.

The role emphasizes leadership, collaboration with healthcare partners, and ongoing staff development to deliver high-quality, compassionate end-of-life care.

Qualifications

• Education and/or Experience: A.S. degree required; Bachelor’s degree from four-year college or university and Master’s degree preferred.
• Two years Hospice experience with at least two years of management experience preferred.
• At least five years total years experience as an RN preferred.

Responsabilités

• Provide daily leadership, oversight, and support to the Access Team staff within the Admission–Care Navigation Department.
• Serve as the primary liaison to hospitals, communicating Empath Health’s Mission, Vision, and values to partners and stakeholders.
• Ensure compliance with all state and federal regulations, as well as standards of care established by accrediting agencies.
• Coordinate and evaluate patient access and care navigation processes to promote quality, efficiency, and continuity of care.
• Monitor performance and support staff development through coaching, training, and performance feedback.
