Job summary

Empath Health in Florida is seeking an RN Case Manager to provide comprehensive care coordination and case management. This role involves obtaining patient history, performing age-specific assessments, and developing and updating a Plan of Care with the patient, family, IDT, attending physician, and partnering facility.

You will supervise LPNs and Home Health Aides, evaluate services biweekly, and ensure medication administration and patient education are carried out according to policy.

Qualifications

• Associate degree or equivalent required.
• Valid Florida RN license required.
• Valid Florida driver’s license required.
• Minimum of one year related experience and/or training or equivalent.
• Bilingual Spanish/English preferred.
• Reliable transportation for field-based tasks.
• Proof of valid automobile insurance to be placed in HR file.

Responsibilities

• Obtain patient history and perform age-specific assessments, initiating appropriate referrals and collaborating with IDT, physician, and partnering facility.
• Develop and update Plan of Care with patient, family, IDT, attending and physician; monitor progress toward goals.
• Direct supervision of LPN and Home Health Aide per agency policy; evaluate services biweekly with on-site supervisory visits.
• Administer, monitor and document medications as prescribed; provide patient/family teaching.
