Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking a Hospital Access Navigator to lead the Access Team within the Admission–Care Navigation Department in Hillsborough County, Florida.

You will liaison with hospitals, ensuring compliance with regulations and promoting efficient patient access and care navigation across multiple locations. RN licensure and hospice leadership experience are required or preferred.

Qualifications

• A.S. degree required; Bachelor’s degree from four‑year college encouraged; Master’s degree preferred.
• Two years hospice experience with at least two years of management experience preferred.
• At least five years total RN experience preferred.
• RN licensure is required.
• Must have reliable transportation and travel between multiple locations.

Responsabilités

• Provide daily leadership, oversight, and support to the Access Team within the Admission–Care Navigation Department.
• Serve as the primary liaison to hospitals, communicating Empath Health’s Mission, Vision, and values to partners and stakeholders.
• Ensure compliance with state and federal regulations and accrediting standards.
• Coordinate and evaluate patient access and care navigation processes to promote quality and continuity of care.
• Monitor performance and support staff development through coaching, training, and feedback.
