Job summary

Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking a Hospital Access Navigator to lead the Access Team within the Admission–Care Navigation Department in Hillsborough County, Florida.

You will liaison with hospitals, ensuring compliance with regulations and promoting efficient patient access and care navigation across multiple locations. RN licensure and hospice leadership experience are required or preferred.

Qualifications

• A.S. degree required; Bachelor’s degree from four‑year college encouraged; Master’s degree preferred.
• Two years hospice experience with at least two years of management experience preferred.
• At least five years total RN experience preferred.
• RN licensure is required.
• Must have reliable transportation and travel between multiple locations.

What You’ll Need

• Education and/or Experience: A.S. degree required - Bachelor’s degree (B.A.) from four-year college or university and Master’s degree preferred.
• Two years Hospice experience with at least two years of management experience preferred.
• At least five years total years experience as an RN preferred.
• Licensure: RN
• Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file. This position requires travel between multiple locations.
