Job summary

i4 Search Group is seeking a compassionate Nurse Practitioner or Physician Assistant to join our post-acute care team in Clarksville/South Boston, VA. You will collaborate with physicians, nursing staff, and medical directors to deliver high-quality care on a Monday-Friday schedule with on-call rotation.

The role emphasizes independent practice within a supportive team, with emphasis on geriatrics and general internal medicine, EMR documentation, and ongoing professional development.

Qualifications

• Active NP or PA license and DEA certification.
• At least 1 year of NP/PA experience.
• Geriatrics, Internal Medicine, ICU, ER, Hospitalist, or General Practice background valued.
• Strong organizational and interpersonal skills.
• Ability to work independently while collaborating with a team.

Responsibilities

• Coordinate and deliver patient care in post-acute facilities.
• Collaborate with nursing staff, physicians, and facility leadership to optimize outcomes.
• Conduct patient rounds, write orders, and manage diagnostic and therapeutic procedures.
• Document daily visit notes in an EMR system (dictation services available).
• Maintain accurate professional records, billing, and reporting.
• Provide care in alignment with facility policies, bylaws, and healthcare regulations.
