Job summary

AlignMed Partners seeks an Advanced Practice Provider (NP or PA) to deliver primary and urgent care to nursing center residents in Newport, VT. This part-time role requires collaboration with Center Leadership and medical directors to ensure high-quality care and alignment with company goals.

New grads are welcome; 2+ years in adult/geriatric care preferred. A nursing license and NP/PA certification are required, with state licensure and NAB/DEA as applicable.

Qualifications

• Graduate from an accredited School of Nursing; licensed RN.
• Certified as a Nurse Practitioner with MSN and Older Adult and/or FNP concentration.
• PA must be ARC-PA accredited and pass the PANCE; PA licensure and NCCPA certification.

Responsibilities

• Collaborates on-site with Center Leadership to meet goals and priorities.
• Provides primary and urgent care to nursing center residents; coordinates with Medical Director/Attending Physician.
