What You'll Be Doing
We are looking for a part-time Weekend Nurse Practitioner (NP) to provide on-site clinical coverage at a skilled nursing facility on Saturdays and Sundays from 9:00 AM–2:00 PM.
This role is focused primarily on evaluating and managing acute clinical issues that arise over the weekend , helping residents receive timely treatment in the facility whenever appropriate and avoiding unnecessary emergency department visits or hospital transfers.
This is a highly independent role with physician support available as needed. It is a great fit for an experienced clinician looking for a consistent, predictable weekend schedule without overnight call.
Key Responsibilities
- Evaluate and treat residents with new or worsening acute medical concerns during weekend coverage hours.
- Respond to clinical issues identified by nursing staff and determine the appropriate evaluation, treatment, and follow-up plan.
- Manage common acute conditions in the skilled nursing setting, including infections, changes in mental status, respiratory symptoms, falls, pain, dehydration, and other changes in condition.
- Determine when residents can safely be treated in the facility and when escalation to the emergency department or hospital is appropriate.
- Review relevant laboratory results, imaging, medications, and recent clinical history to guide treatment decisions.
- Communicate clearly with nursing staff, physicians, residents, and families regarding changes in condition and treatment plans.
- Provide follow-up for residents recently discharged from the hospital or experiencing a significant change in condition when clinically appropriate.
- Maintain timely, accurate, and compliant documentation of all patient encounters.
- Collaborate with the facility and Fortis Care clinical team to ensure appropriate handoff and continuity of care following the weekend.
Skills & Abilities
- Strong clinical judgment in adult and geriatric medicine.
- Comfortable evaluating and managing acute medical problems independently.
- Ability to quickly assess changes in condition and determine the appropriate level of care.
- Excellent communication and collaboration skills with nursing staff, patients, families, and physicians.
- Strong organizational and documentation skills.
- Comfortable working independently in a skilled nursing facility with physician support available as needed.
- Commitment to high-quality, patient-centered care and reducing avoidable hospitalizations when clinically appropriate.
Qualifications
- Current Nurse Practitioner license in the state of Georgia.
- Board certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or other applicable NP certification.
- Unrestricted DEA registration.
- Minimum 1–2 years of Nurse Practitioner experience preferred.
- Experience in skilled nursing, post-acute care, geriatrics, internal medicine, hospital medicine, urgent care, or emergency medicine strongly preferred.
- Familiarity with EHR systems and clinical documentation best practices.
