The full-time role offers Monday–Friday, 9:00 AM–5:00 PM with no weekends or call responsibilities, and base facilities-based rounding in post-acute care settings. Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and unrestricted NP or PA license in the applicable state or eligibility for licensure.
• National board certification as a Nurse Practitioner or Physician Assistant.

Responsibilities

• Provide patient-centered care during transitions from hospital to post-acute facilities.
• Collaborate with interdisciplinary teams to manage chronic and acute conditions.
• Perform facility-based rounding in post-acute care settings.
• Coordinate care to reduce hospital readmissions and improve outcomes.
