The NP will manage chronic and urgent conditions, order tests, and collaborate with an interdisciplinary team to ensure high-quality patient outcomes. A supportive practice uses mobile-based EHR and on-call duties are limited.

Qualifications

• Virginia NP license required.
• DEA registration required.
• 1+ year advanced practice experience.
• 1+ year experience in facilities/home care/geriatrics.

Responsibilities

• Plan, direct, and evaluate medical care.
• Examine and treat chronic illnesses.
• Examine and treat urgent illnesses.
• Order and interpret laboratory and diagnostic testing.
• Collaborate with caregivers and other healthcare professionals.
• Work closely with the interdisciplinary care team.
• Make referrals as necessary.
