Job summary

Legacy Care is seeking a dedicated Nurse Practitioner to provide patient-centered care in post-acute settings in Richmond, Virginia. This weekend opportunity allows you to contribute to patients’ health while collaborating with a multidisciplinary team.

You will assess health needs, develop care plans, and work with facility staff to ensure continuity of care throughout the patient journey. EMR documentation is essential for accurate follow-up.

Qualifications

• Master’s or Doctoral NP degree from an accredited program.
• Active, unrestricted Virginia NP license.
• Current DEA registration.
• National board certification in the relevant population focus.
• Experience in post-acute care, long-term care or geriatrics preferred.
• Proficiency with EMR systems and strong clinical documentation.
• Strong clinical judgement, communication and interpersonal skills.
• Ability to work autonomously with a collaborative team.
• A compassionate, patient-centered approach to care.

Responsibilities

• Provide high-quality, patient-centered care in post-acute settings.
• Assess health needs, develop care plans and monitor progress.
• Order and interpret tests, prescribe treatments, refer as appropriate.
• Collaborate with nursing staff, physicians and other clinicians.
• Communicate clearly with patients and families about plans.
• Document assessments and follow-up plans in EMR promptly.
• Support care coordination and transitions between settings.
