Job summary

Vertical Home Health is seeking a Care Coordinator to provide comprehensive oversight of client care coordination, ensuring continuity, compliance, and high-quality support.

You will maintain accurate, timely documentation, update service plans as clients' needs evolve, and act as a hub between clients, caregivers, case managers, and agency leadership to promote clarity and responsive, person-centered care.

Maintains accurate, timely documentation and ensures service plans are updated to reflect each client’s evolving needs. Serves as a key communication hub between clients, caregivers, case managers, and agency leadership, promoting clarity, responsiveness, and person‑centered care. Actively monitors client satisfaction, resolves concerns, and supports smooth delivery of authorized services while upholding all regulatory and agency standards.

The Coordinator also proactively identifies risks to client retention or service continuity and works across departments to resolve barriers before they result in missed care, client dissatisfaction, or discharge.
