Job summary

Personal Touch Home Health Care Services Inc. is seeking a Care Coordinator to assess, plan, and coordinate services for children under the CLTS Waiver. You will work with families and providers to create individualized plans and ensure timely service delivery.

The role emphasizes family-centered, trauma-informed care, with travel in the community and opportunities to participate in trainings and team meetings. A background in health/human services and basic EHR skills are required.

Qualifications

• Bachelor’s degree in a health or human services field (e.g., Social Work, Psychology, Counseling).
• At least 1 year of experience working with children in the target population.
• Strong communication, organizational, and documentation skills.
• Proficiency with electronic health records and computer applications.
• Valid driver’s license and reliable transportation; ability to respond in crisis situations.
• Bilingual fluency in Spanish is preferred but not required.
