Job summary

PA Health Management seeks a Service Coordinator to support participants in Medicaid Waiver programs and related services. The role blends remote administrative work with field-based participant support to promote independence and quality of life.

You will collaborate with providers, agencies, and interdisciplinary teams to ensure compliant, person-centered planning, accurate documentation, and timely service coordination that meets regulatory and funding requirements.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field is required.
• Minimum of three (3) years of experience in service coordination or case management.
• Experience with waiver-funded or I/DD populations strongly preferred.
• Proficiency in Microsoft Office Suite and data entry with strong documentation.

Responsibilities

• Conduct person-centered assessments and annual level-of-care evaluations.
• Develop, implement, and update individualized service plans quarterly or as needed.
• Coordinate services with healthcare providers, community agencies, and government entities.
• Enter all required documentation accurately and within established timelines.
