People Incorporated Mental Health Services is hiring Case Manager II for the Waiver Case Management team in St. Louis County, MN.

The role focuses on client-centered planning, service authorization, and community integration, with an autonomous, hybrid schedule after training. Responsibilities include attending bi-annual client meetings, assessing needs, coordinating with partners, and maintaining detailed records in the Support Plan.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or closely related field.
• Or Bachelor's degree in any field plus one year of social work/case management experience.
• Must reside within the metro area.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain collaborative relationships with clients and partners.
• Assess client needs and ensure continuity of care.
• Make referrals and authorize services based on client needs.
• Document all interventions and services in the Support Plan.

Education

Bachelor’s Degree in social work, psychology, sociology or closely related field

Bachelor’s Degree in any field + 1 year social worker/case manager experience
