Job summary

People Incorporated Mental Health Services in Duluth, MN is seeking a Case Manager II for Waiver Case Management to support clients diagnosed with chronic mental illness in a hybrid, full-time role.

You will develop person-centered Support Plans, authorize services, monitor progress, maintain records, and collaborate with partners to ensure continuity of care. The position offers a stable schedule, a $24.25–$25.75 hourly wage, and a $1,000 hiring bonus for external candidates.

Qualifications

• Bachelor’s Degree in social work, psychology, sociology or closely related field.
• At least one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain collaborative relationships with clients and demonstrate respectful communication.
• Assess client needs and ensure continuity of care.
• Maintain regular contact with collaborative partners.
• Make referrals and authorize services based on client needs and outcomes.
• Maintain records and document all services provided.
