Affinity Care MN is hiring a mission-driven Waiver Case Manager to deliver person-centered case management for individuals with serious mental illness and co-occurring conditions. The role supports clients whether in their own homes or residential settings, with a focus on improving quality of life and empowering clients through knowledge and resources.

Qualifications

Bachelor's degree in social work, psychology, sociology, or related field OR a bachelor's in any field plus one year of social work/case management experience.
Willingness to travel throughout the Metro Area with a valid driver’s license and insured vehicle.
Maintain an acceptable motor vehicle record and complete orientation and training.

Responsibilities

Carry and maintain a caseload of individuals with mental illness, substance use disorders, and co-occurring conditions.
Meet with clients monthly and twice yearly per waiver rules; manage intake and discharge within parameters.
Collaborate with physicians, mental health providers, and care teams to update client plans.
Ensure financial authorizations and provider contracts are in place; document services in records.
Connect clients with community/housing resources and provide crisis assessment/intervention.
