Salary ranges from $24.25 to $25.75 per hour, with a $1,000 hiring bonus for external candidates. Responsibilities include functional assessments, plan development, regular plan reviews, referrals, and thorough documentation.

Qualifications

• Bachelor’s degree in a related field and 2,000 hours of experience in education or treatment of persons with developmental disabilities or related conditions.
• Experience in case management and ability to assess client needs and coordinate services.

Responsibilities

• Perform functional assessments that lead to individual plan development and access to mental health and related services.
• Conduct regular reviews of plan effectiveness and adjust services to maximize support.
• Assess client needs and ensure continuity of care.
• Make ongoing referrals to resources based on assessment and current client needs.
• Maintain daily records for continuity of service and reporting to funding agencies.
• Document all interventions and relevant information to the treatment plan.
• Attend client monthly meetings to monitor services.
