Job summary

• Ability to develop and monitor person-centered support plans.
• Experience coordinating services and referrals for complex cases.

Responsibilities

• Attend bi-annual client meetings to monitor services.
• Develop and maintain collaborative client relationships.
• Assess client needs and ensure continuity of care.
• Make referrals and authorize services based on client needs.
• Maintain records and document all services.
