Qualifications

• Bachelor’s degree in social work, psychology, sociology, or closely related field.
• Or Bachelor's degree in any field plus one year of social work/case management experience.
• Must reside within the metro area.

Responsibilities

• Attend bi-annual meetings with clients to monitor services.
• Develop and maintain collaborative relationships with clients and partners.
• Assess client needs and ensure continuity of care.
• Make referrals and authorize services based on client needs.
• Document all interventions and services in the Support Plan.

Case Manager II - Waiver Case ManagementPeople Incorporated is seeking Case Manager II to join our Waiver Case Management (St. Louis County) team to work with clients who are diagnosed with chronic mental illness or other ongoing health conditions. We connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management Our Case Managers see clients twice annually and develop long term relationships by developing dynamic service plans. Our staff operate autonomously, creating their own hybrid schedules after completing initial training.
