Job summary

People Incorporated Mental Health Services in Minnesota is seeking a Case Manager II for Waiver Case Management in Ramsey County. The role requires hybrid on-site work in the Twin Cities area with full-time hours and a $24.25–$25.75 hourly range.

The team focuses on coordinating services for clients with chronic mental illness, developing person-centered Support Plans, and ensuring integrated community living.

Qualifications

• Background and requirements screening for health care programs
• Valid driver’s license and auto insurance required
• Good driving record and current auto insurance

Responsibilities

• Attend bi-annual meetings with clients to monitor services
• Develop and maintain collaborative relationships with clients
• Assess client needs and ensure continuity of care
• Maintain regular contact with collaborative partners
• Make referrals and authorize services based on client needs
• Document all services and maintain records
• Document interventions and relevant information for the Support Plan

Education

Bachelor’s Degree in behavioral science or related field

Master’s Degree in behavioral science or related field

Bachelor’s Degree in Social Work / Psychology / Sociology or related field

One year of experience as a social worker / case manager / care coordinator
