Job summary

Residential Transitions is seeking a Case Management Case Aide to provide administrative and coordination support for the Waiver Case Management program across Minnesota counties. You will help with documentation, scheduling, and communication within care teams.

You will ensure timely, accurate records, support billing processes, and collaborate with case managers to deliver high-quality, person-centered services while complying with DHS and HIPAA standards.

Qualifications

• High school diploma or equivalent.
• At least 1 year of case aide or administrative experience, or 1 year of post-secondary study.
• Willingness to travel in the Metro Area with a valid license and insured vehicle.
• Valid driver's license, reliable vehicle, and auto insurance required.
• Must pass MN DHS background check to provide services.
• Must pass OIG screening for federal health programs eligibility.

Responsibilities

• Provide administrative support to the Waiver Case Management program across counties.
• Assist with case prep, filing, and documentation maintenance.
• Schedule client meetings and care-related appointments.
• Enter data for screenings and service authorizations.
• Coordinate provider and vendor services for coordination.
• Attend client appointments as needed.
• Communicate with clients and families to support administrative needs.
• Ensure DHS and HIPAA compliant documentation.
