Job summary

Local Infusion is seeking a skilled Nurse Practitioner licensed in Maine to join our virtual care team in a remote, patient-facing role focused on neurology and memory care. You will deliver care via telehealth and collaborate with physicians to increase access and timely treatment.

In this role you will perform patient assessments, develop care plans, prescribe therapies when appropriate, and educate patients and families.

Qualifications

• Active, unrestricted NP license in Maine.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia care.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment with supervising physicians.
• Prescribe therapies as appropriate and ensure continuity of care in collaboration with neurologists.

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Maine to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

• Active, unrestricted NP license in Maine; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
