Job summary

Local Infusion is seeking a licensed Nurse Practitioner in New Hampshire to join our remote, patient-facing neurology and memory care team. This telehealth role delivers direct clinical care to Local Infusion patients, partnering with physicians for timely treatment.

You will conduct assessments, develop care plans, educate families, document in EMR, and manage a full patient panel with supervision. We offer a remote-first environment with strong benefits and growth opportunities.

Qualifications

• Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers

Responsibilities

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.
