Job summary

Local Infusion is seeking a licensed Nurse Practitioner in Maine for a remote, patient-facing neurology and memory care role. You’ll provide telehealth-based care, work with supervising physicians, and help improve access to treatment.

You’ll assess patients, create care plans, and document thoroughly while educating patients and families about diagnoses and progression. Strong collaboration with the care team is essential.

Qualifications

• Active, unrestricted NP license in Maine; geriatrics, adult-gerontology, or family practice certification preferred.
• 2+ years of clinical experience in neurology, memory care, or dementia care settings.
• Excellent clinical judgment and documentation skills.

Role Overview: Nurse Practitioner, Neurology and Memory Care

We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Maine to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies
• Partner with our neurologist(s) and broader care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

Key Accountabilities

• Active, unrestricted NP license in Maine; certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgement and clinical documentation skills
• Comfortable managing a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
