Job summary

SSTAR’s Fall River Women’s Center is seeking a Victim Services Case Manager to provide trauma-informed casemanagement, advocacy, crisis intervention, safety planning, and resource coordination to adult victims and survivors of crime.

The role partners with survivors across medical, behavioral health, housing, legal and financial services, to support safety, stability and recovery, with services offered in office, community and healthcare settings.

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Criminal Justice, Counseling, or related field required.
  • One year of experience providing case management, victim advocacy, behavioral health, crisis intervention, or related human services preferred.
  • Experience working with survivors of trauma, domestic violence.

Responsibilities

  • Provide trauma-informed casemanagement, advocacy, crisis intervention, safety planning, and resource coordination for adult victims and survivors of crime.
  • Serve as a consistent point of contact to reduce barriers to care and advocate across medical, legal, housing, and social service systems.
  • Coordinate referrals to housing, healthcare, legal, benefits, and community resources and document progress.

Join SSTAR’s Fall River Woman's Central Team as a Case Manager.

The Victim Services Case Manager provides trauma-informed casemanagement, advocacy, crisis intervention, safety planning, andresource coordination to adult victims and survivors of crime. Workingwithin the Women's Center, this position helps survivors achievesafety, stability, and recovery by connecting them with medical,behavioral health, housing, legal, financial, and community resources.

The Case Manager serves as a consistent point of contact,reducing barriers to care while advocating for survivors throughoutmedical, legal, housing, and social service systems. Services may beprovided in the office, community, healthcare settings, and othersafe locations as appropriate.

Experience and Skills:

Direct Client Services

  • Conduct trauma-informed needs assessments.
  • Develop individualized service plans in collaboration with survivors.
  • Provide ongoing case management focused on safety, stabilization,and recovery.
  • Complete individualized safety planning and regularly update plansbased on changing circumstances.
  • Respond to referrals in a timely manner and engage survivors usingtrauma-informed practices.
  • Maintain regular contact with survivors to monitor progress towardidentified goals.

Advocacy

  • Advocate on behalf of survivors with healthcare providers, housingagencies, employers, schools, and community organizations.
  • Accompany survivors to medical appointments, housing appointments,and other meetings as appropriate.
  • Assist survivors in understanding available community resources.
  • Coordinate with Women's Center Team members.

Case Management (build this in first 90 days)

Coordinate referrals and link survivors with:

  • Transitional housing
  • Medical and behavioral h ealth care
  • Substance use treatment
  • Food and basic needs
  • Employment and vocational services
  • Transportation
  • Legal services
  • Immigration resources
  • Public benefits
  • Community support programs

Assist survivors with completing applications and requireddocumentation while following referrals through to resolution whenever possible.

Community Collaboration

  • Develop and maintain collaborative relationships with community providers.
  • Participate in multidisciplinary case coordination meetings.
  • Collaborate with SSTAR programs including Behavioral Health,Health Center, Connect & Protect, SAFEPLAN, Housing, andRecovery Services.
  • Represent the Women's Center at community meetings and outreach events.
  • Assist with community education and prevention activities.

Documentation and Compliance

  • Maintain accurate, timely, and complete documentation within theapproved database for data gathering purposes.
  • Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2,MOVA requirements, and agency policies.
  • Participate in supervision, case consultation, and required trainings.
  • Maintain knowledge of victim rights, trauma-informed care,community resources, and best practices.

General:

  • Complete all required documentation, checklists, and shiftcommunication accurately and on time
  • Follow incident reporting procedures and immediately report safetyor compliance concerns
  • Maintain confidentiality and uphold patient rights, dignity, andprivacy at all times
  • Attend required trainings, supervision, and meetings
  • Maintain awareness of agency communications and policies
  • Other duties as assigned

EDUCATION: Bachelor's degree in Social Work, HumanServices, Psychology, Criminal Justice, Counseling, or related field required.

WORK EXPERIENCE:

  • One year of experience providing case management, victim advocacy,behavioral health, crisis intervention, or related human services preferred.
  • Experience working with survivors of trauma, domestic violence.
  • Strong crisis intervention and de-escalation skills.
  • Knowledge of community resources and service systems.
  • Ability to work independently and as part of a multidisciplinary team.
