Job summary

SSTAR's Fall River Women's Center is seeking a compassionate Case Manager to provide trauma-informed case management, advocacy, crisis intervention, safety planning, and resource coordination for adult victims and survivors of crime. You will connect clients with medical, behavioral health, housing, legal, financial, and community resources in office, community, and healthcare settings.

The Case Manager serves as a consistent contact, reducing barriers to care while advocating for survivors

Qualifications

• Direct Client Services: conduct trauma-informed needs assessments, develop service plans, provide ongoing case management for safety, stabilization, and recovery.
• Advocacy: advocate with healthcare, housing, employers, schools, and community orgs; accompany to appointments.
• Case Management: coordinate referrals to housing, medical/behavioral health, legal, benefits; document progress.
• Documentation & Compliance: HIPAA/42 CFR Part 2/MOVA compliant documentation; supervision and trainings.

Responsibilities

• Coordinate referrals to transitional housing, medical/behavioral health care, substance use treatment, food and basic needs, employment and transportation, legal services, immigration resources, public benefits, and community programs.
• Develop and maintain collaborative relationships with community providers; participate in multidisciplinary meetings.
• Represent the Women's Center in community events and outreach; assist with education and prevention activities.
• Maintain confidentiality and uphold patient rights and dignity; follow incident reporting procedures.
