Job summary

SSTAR’s Fall River Inpatient Program is seeking an entry-level Case Manager to support patients as they transition from inpatient care to continued treatment and community resources.

You’ll work with a collaborative team to ensure individuals have the tools and connections they need for success, including housing and ongoing treatment options.

Qualifications

• High school diploma or GED required; Bachelor's degree in Social Work or related field preferred.
• Strong verbal and written communication skills.
• Open to working with people from diverse backgrounds, cultures and lifestyles in a non-judgmental manner.
• Direct care positions in addiction programs require two years of continuous sobriety for individuals in recovery.

Responsibilities

• Provide comprehensive case management services focused on aftercare planning.
• Connect clients with ongoing treatment, housing, and community resources.
