Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Case Manager to provide day-to-day support for veteran households enrolled in the SSVF program. You will coordinate outreach, assess housing needs, and link clients to permanent housing with wraparound supports.

The role emphasizes collaboration with VA and community partners, documentation in HMIS, and adherence to program guidelines. Travel within the Carolinas and surrounding areas may be required.

Qualifications

• Bachelor's degree in counseling psychology, rehabilitative counseling, or related human-service field preferred.
• Experience working with homeless individuals and families, including veterans, is a plus.
• Valid driver's license and the ability to drive per agency policy.
• CPR/First Aid certification must be acquired and maintained.
• Must be willing to travel to housing sites and in the Carolinas as needed.

Responsibilities

• Coordinate outreach for the SSVF program including establishing an outreach plan to target participants and visiting VA facilities, shelters, and other sites.
• Screen program participants for eligibility using VA-approved forms and verify information.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners.
• Travel to housing sites to monitor progress and address health, behavioral or life-skills issues.
• Document services in HMIS and complete required reporting.
