Responsibilities

• Coordinate outreach for the SSVF program with VA, shelters, and community providers.
• Screen program participants for eligibility using VA-approved forms and verify information.
• Conduct housing and supportive services assessments and develop Family Services Plans.
• Provide service coordination between the program and community partners and revise service plans as needed.
• Arrange housing support and wraparound services to support transition to permanent housing.
• Travel to housing sites to monitor progress and address health, life skills, or housing-related issues.
• Maintain confidential communication with the client service team and complete required reporting.
