Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Case Manager in Rocky Mount, NC to provide day-to-day support for participating veteran households and coordinate services through the SSVF program.

The role emphasizes outreach, housing stabilization, and wraparound supports, collaborating with the VA and community partners to help clients attain or maintain permanent housing in their home community.

Qualifications

• A bachelor's degree in counseling psychology, rehabilitative counseling, or related human service field is preferred (or comparable experience).
• Experience working with homeless individuals and families including veterans is a plus.

Responsibilities

• Coordinate outreach efforts for the SSVF Program including establishing outreach plans to target participants and visiting VA, shelters, and other places veterans and their families congregate.
• Screen potential program participants for program eligibility using VA-approved forms and verify information.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners and revise service plans as needed.
• Provide housing support and wraparound services including connections to VA and mainstream resources to support transition to or maintenance of permanent housing.
