Job summary

Volunteers of America Chesapeake & Carolinas seeks a Case Manager for the SSVF program in Durham, NC. The role provides day-to-day support and services to veteran households, with emphasis on homelessness prevention, rapid re-housing and wraparound supports via intake, assessment and connections to VA resources and community partners.

You will coordinate outreach, conduct eligibility screening, develop comprehensive service plans, and ensure coordinated care, documentation and HMIS reporting.

Qualifications

• A bachelor's degree in counseling psychology, rehabilitative counseling, or related human service field is preferred (or comparable experience).
• Experience working with homeless individuals and families including veterans is a plus.

Responsibilities

• Coordinate outreach efforts for the SSVF Program including establishing outreach plans and VA/community partner engagement.
• Screen potential program participants for eligibility using VA-approved forms and verify information including veteran and income status.
• Conduct housing and comprehensive supportive services assessments and develop a Family Services Plan.
• Provide service coordination between the program and community partners for participants and families.
• Travel to housing sites to support participants and monitor progress.
