Volunteers of America Colorado is seeking a Field Case Manager for the Back Home SSVF program, a rapid rehousing initiative helping veterans experiencing homelessness or at risk. This role centers on housing-focused case management and wrap-around services to connect veterans with VA benefits, health referrals, and financial resources.

The role involves needs-based assessments, service coordination, and support toward housing stability while meeting credentialing requirements and maintaining

Qualifications

• Four years of direct experience in the provision of human services/related experience or education.
• Must possess a Colorado driver’s license and state-mandated automobile insurance.
• Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

• Direct and facilitate delivery of appropriate support services for participant households.
• Conduct needs-based and program eligibility assessments.
• Engage in community outreach and provide case management services.
• Coordinate program-wide services and ensure progress toward knowledge and practices.
