Job summary

Volunteers of America Chesapeake & Carolinas seeks a Case Manager to support veteran households across NC. You will link intake to permanent housing, coordinate with VA and community partners, and promote rapid rehousing with wraparound services.

The role requires outreach, housing assessments, and collaboration to ensure timely services and housing stability for veterans and their families.

Qualifications

• Bachelor’s degree in counseling psychology, rehabilitative counseling, or related human-service field preferred.
• Experience working with homeless individuals and families including veterans is a plus.
• Strong linkages with VA and community partners preferred.

Responsibilities

• Coordinate outreach and engage veteran families for SSVF program.
• Conduct housing assessments and develop Family Services Plans.
• Provide case management and wraparound supports with VA resources.
• Travel to housing sites and maintain HMIS documentation.
