Qualifications

• Four years of direct experience in the provision of human services/related experience or education.

Responsibilities

• Direct and facilitate delivery of support services as indicated by service modalities.
• Conduct needs-based and program eligibility assessments; engage in outreach; provide case management; coordinate services program-wide.
