Job summary

Access in Medford, OR is seeking a Veteran Services Lead Housing Stabilization Case Manager. The role works with a multi-disciplinary team to help participants access housing resources, navigate programs, and build life skills for self-sufficiency. The position requires travel between office, homes, and outreach activities.

You will supervise staff, ensure eligibility compliance, and coordinate with Veterans Services to deliver high-quality support and crisis response as needed.

Qualifications

• Minimum 2 years of case management experience.
• Ability to engage with participants from diverse backgrounds.
• Excellent interpersonal, written and oral communication skills.
• Proficient in Microsoft Office and web-based applications.
• Valid Oregon driver’s license.

Responsibilities

• Lead the day-to-day task organization within Veterans Services program.
• Assist the Program Supervisor with training and monitoring Housing Stabilization Case Managers.
• Interview and screen participants to determine eligibility for Federal and State assistance programs.
• Lead By Name List for Jackson County with community partners and keep it up to date.
• Ensure veteran participant caseloads match staff availability and provide coverage.
• Review participant files regularly to confirm eligibility.
• Monitor work of Community Support Specialists and related activities.
• Provide support in case management, peer support and outreach.
• Maintain minimum caseloads and advocate for participants’ housing goals.
• Provide crisis intervention and connect participants to resources as needed.
• Represent the Veteran By Name list and Veteran Subcommittee meetings.
