Job summary

ACCESS is seeking a Housing Stabilization Case Manager II (HSCM, Healthcare Navigation) to assist Veterans and their families in Medford, Oregon. The role combines office-based work with community/home visits to provide wrap-around services and coordinate with a multidisciplinary team.

You will maintain high-acuity caseloads, screen for eligibility for federal/state programs, and support healthcare-related stabilization.

Qualifications

• Belief in ACCESS’ mission to provide food, warmth, and shelter.
• At least 2 years of case management experience with participants.
• Ability to manage high-acuity Veteran caseloads with discretion.
• Excellent interpersonal, written, and oral communication.
• Proficiency in Microsoft Office and web-based tools.

Responsibilities

• Maintain a minimum caseload of high-acuity Veteran households.
• Interview, screen, and determine eligibility for Federal/State assistance.
• Support Veterans stabilization plans related to healthcare needs.
• Help households set short- and long-term housing/health goals.
• Document progress and ensure accurate data entry in systems.
• Refer participants to ACCESS programs and external resources.
• Maintain participant files and filing systems per criteria.
