Job summary

USVETS Houston is seeking a Case Manager II to guide veterans toward independence through comprehensive case management, intake, and coordination with VA and community partners. The role emphasizes timely documentation, IAP development, and data entry in HMIS and related systems.

The Case Manager II will conduct home visits, track progress, support life skills groups, and participate in team meetings while handling multiple program-specific duties across housing and workforce services.

Responsibilities

• Assess veterans referred to the program across housing, employment, education, finances, health, legal, family and medical needs.
• Coordinate care with VA and community partners; prepare eligibility documentation.
• Facilitate intake, maintain confidential client files, and complete Individual Action Plans (IAP).
• Document progress notes in DAP format and update IAPs monthly; prepare discharge summaries.
