Job summary

USVETS Orange County is seeking a Case Manager II to guide veterans toward independence. The CM II assesses needs across housing, employment, health, and legal areas, coordinates with VA and community partners, and maintains confidential client files and IAPs.

You will conduct home visits, document progress in HMIS, facilitate life skills classes, and ensure program compliance while reporting to the Program Coordinator.

Qualifications

• Bachelor’s Degree required; degree in Social Work, Psychology, Counseling, or related field preferred.
• Strong oral and written communication skills.
• Proficiency in Microsoft Office 365 apps, email, and internet research.
• Valid driver’s license; meet insurance requirements and complete driver training.

Responsibilities

• Assess veterans referred to the program across housing, employment, education, financial, health and legal needs.
• Coordinate care with VA personnel and community partners for eligibility and services.
• Maintain confidential client files and ensure all forms are current and properly filed.
• Document progress in IAPs and DAP format; prepare discharge summaries.
