Responsibilities include home visits, progress documentation, discharge summaries, and delivering life skills groups tailored to veteran subpopulations.

Qualifications

  • Bachelor’s degree in social services or related field required.
  • Strong oral and written communication skills.
  • Ability to work independently and in a team.
  • Valid driver’s license required.

Responsibilities

  • Assess housing, employment, education, financial, and health needs of veterans.
  • Coordinate care with VA and community partners for eligibility and services.
  • Facilitate intake, compile client files, and maintain confidential records.
  • Develop Individual Action Plans (IAP) with clients.
  • Conduct home visits to provide case management and verify welfare.
  • Prepare discharge summaries and maintain HMIS data.
  • Deliver life skills classes and specialized case management as applicable.
