Job summary

Catholic Community Services of Western Washington is seeking a Case Manager for the Veterans Services program. You will manage a caseload across nine counties, perform intakes, assess housing needs, and coordinate housing stability plans with VA and community resources.

The role emphasizes outreach to eligible Veterans, data entry into HMIS, collaboration with the Veterans Program Manager, and ensuring timely payments and referrals.

Qualifications

• Post-secondary education in social services or 2 years relevant experience.
• Experience serving low-income/no-income households.
• Experience in case management.
• Strong communication and teamwork.
• Ability to use HMIS and MS Word.

Responsibilities

• Complete thorough intakes and eligibility screening with referred clients.
• Develop and update Housing Stability Plans with clients.
• Provide outreach to Veterans and build community connections.
• Enter client data into HMIS and maintain records.
• Coordinate with landlords and manage rent assistance processes.
