Qualifications

• Bachelor's degree preferred; high school diploma or GED required.
• 2+ years of case management experience; 1-3 years minimum; social services experience preferred.
• Proficiency in Excel; ability to write clear correspondence and reports.
• Valid driver's license; experience working with veterans or outreach roles is a plus.

Responsibilities

• Conduct full assessments with applicants to enroll SSVF-eligible veterans in case management for a minimum of 90 days.
• Ensure rapid entrance into permanent safe housing or maintain stable housing to prevent homelessness.
• Mediate, advocate, and leverage support for eligible veterans, referring to healthcare navigation, legal, employment, financial, and other services.
• Identify and connect veterans to community resources for overall well-being, including emergency housing needs.
• Maintain accurate records of resources provided and manage client files with confidentiality.
• Represent Baker Ripley and the Veteran Services team professionally in meetings and events.
• Collaborate with other agencies to provide a comprehensive case management process.
• Perform other duties as assigned.
