Job summary

JVS SoCal is seeking a dedicated SSVF Case Manager to join our Veterans Service Team in Los Angeles County. You will provide housing relocation and stabilization services, linking veterans to community resources and other service providers.

The role emphasizes client-driven support, problem-solving, and person-centered housing advocacy. You will assess housing barriers, coordinate services, maintain HMIS data, and help veterans achieve independent living.

Qualifications

• BA/BS in Social Work or related field (may be substituted for experience)
• Four years of case management or housing navigation experience may be substituted for the educational requirement
• Passion for working with Veterans
• Customer service and counseling expertise to multitask
• Excellent communication skills
• Timeliness and attention to detail
• Must take initiative and work as part of a team toward goals
• Three or more years of experience with priority populations
• Background in Case Management or Career Counseling; prior military and homeless services preferred
• Proficient in Microsoft Office applications (Outlook, Word, Excel, PowerPoint)
• Be assertive, friendly, and willing to work flexible hours
• Reliable transportation to travel between sites
• Occasional evenings and weekend work may be required
• Position is 100% in person (NOT REMOTE or HYBRID)

Responsibilities

• Evaluate housing barriers and social determinant needs of homeless veterans and families
• Provide comprehensive case management support to homeless veterans
• Help articulate housing goals and develop action plans
• Assist veterans in building social skills to secure housing
• Identify and coordinate services for veterans
• Ensure timely, accurate data collection and HMIS reporting
• Determine eligibility for VA HUD VASH/SSVF, DMH, CoC, and other programs
• Assist with housing searches, applications, leases, and tenant rights
• Assist in obtaining decent, affordable housing in integrated settings
• Monitor progress and maintain records in HMIS and paper files
• Maintain HIPAA compliance and funder requirements
• Perform other duties to support program operations
• Meet individual performance goals
