Job summary

JVS SoCal is seeking an SSVF Case Manager as part of the Veterans Service Team to support veterans experiencing homelessness in Los Angeles County. You will provide housing relocation and stabilization services, connect clients with resources, and help them develop skills to live independently.

Responsibilities include assessing housing barriers, coordinating services, and maintaining HMIS data while ensuring HIPAA compliance and teamwork toward program goals.

Qualifications

• BA/BS in Social Work, Career Counseling, or related field; may be substituted for experience.
• Four years of case management or housing navigation experience may substitute the educational requirement.
• Must have passion for working with Veterans.
• Excellent communication skills and ability to multi-task are essential.

Responsibilities

• Evaluate housing barriers and social needs of homeless veterans and families.
• Provide comprehensive case management services to homeless veterans.
• Assist veterans in articulating housing goals and creating action plans.
• Assist veterans in building social skills for housing acquisition and retention.
• Identify, create linkages to, advocate for, and coordinate services for veterans.
• Ensure timely data collection and reporting in HMIS with high data integrity.
