Job summary

STGi is seeking a Case Manager for the HUD-VASH program in Los Angeles, CA. You will provide clinical case management to up to 25 Veterans, conduct assessments, educate households, and develop housing stability plans with strong field/outreach work.

The role requires coordinating with VA and housing partners, delivering mental health and substance abuse counseling within scope, and advocating for veterans to access wrap-around services.

Qualifications

• Knowledge of or experience with DSM diagnoses and evidence-based practices (e.g., Motivational Interviewing, Critical Time Intervention, DBT, Harm Reduction, CBT, Trauma-Informed Care, Housing First, PTSD).
• Ability to work collaboratively in a team.
• Takes initiative with a solution-focused practice.
• Good time management to balance direct service and paperwork.
• Excellent verbal and nonverbal communication skills.
• Must have a current Real ID California driver’s license and insurance for regular driving.

Responsibilities

• Provide direct clinical case management to a caseload of up to 25 Veterans in HUD-VASH.
• Assist navigating the Housing Authority voucher process and secure permanent supportive housing.
• Conduct comprehensive assessments to develop individual case plans.
• Provide education on community living and basic life skills.
• Provide referrals to VA facilities and community services.
• Offer mental health and substance abuse counseling within scope or refer to others.
• Provide crisis management and 24-hour coverage.
• Collaborate with VA and other providers for wrap-around services.
• Advocate to deliver needed services.
• Attend daily/weekly meetings and case conferences.
