Job summary

Insight Housing is seeking a Case Manager to serve Veteran families in the Pathway to Housing program. You will provide housing stabilization, outreach, and case management to help Veterans secure and maintain homes.

You will coordinate VA benefits access, develop action plans, and maintain comprehensive files. A CA driver's license and vehicle are required; a Bachelor’s degree in social services is preferred.

Qualifications

• High school required; Bachelor’s degree in Social Services or related field preferred.
• CPR/First Aid training required.
• 1 year experience in homelessness/mental health or related field.
• Proficient with computer and MS Office; HMIS experience a plus.
• Ability to work independently and as part of a team, with strong communication skills.

Responsibilities

• Coordinate intake and eligibility for new clients and maintain files.
• Provide outreach to homeless Veterans.
• Manage caseload and provide housing and vocational support.
• Develop budgets and service plans with each Veteran client.
• Collaborate with Housing Specialist to maintain landlord relationships.
• Maintain HMIS records and submit program reports.
• Drive to field sites and ensure timely documentation.
