Job summary

Adjoin seeks a dedicated housing case manager to support Veterans in securing permanent housing within California. You will collaborate with clients, coordinate services, and document progress to ensure housing stability and income advancement.

A degree or relevant experience, plus First-Aid/CPR certification, are required, along with a valid driver’s license and vehicle for travel. Responsibilities include HMIS data entry, scheduling, and ongoing referrals, with emphasis on safety and

Qualifications

• Bachelor’s degree or two years of equivalent experience in working with veterans or homelessness.
• Certification in First-Aid and CPR, or willingness to obtain within 45 days.
• Owned vehicle with valid license and insurance, reliable for travel.

Responsibilities

• Collaborate with Veterans and team members to identify and secure affordable housing.
• Provide direct services to a wide range of Veterans and families at risk of homelessness.
• Communicate weekly with each Veteran on the caseload.
• Develop Housing Stability Plans and steps toward permanent housing for Veterans.
• Help Veterans increase income to aid housing stability.
• Maintain accurate case notes and HMIS (Clarity) entries weekly.
