Qualifications

Responsibilities

• Intake assessments to determine eligibility, strengths, and needs.
• Develop, implement, and update individualized service plans with measurable goals.
• Coordinate with internal and external services (mental health, housing, employment).
• Maintain accurate case notes and reports in compliance with HMIS and funding rules.
• Respond to resident crises with de-escalation and referral to emergency services if needed.
• Advocate for resident rights and navigate healthcare, legal, and social services systems.
• Educate residents on life skills, budgeting, job readiness, and wellness.
• Collaborate in team meetings and share information with guardians and partners.
